PROPOSED ADOPTION

Pursuant to Neb. Rev. Stat. § 43-104.14, Lucrece H. Bundy, attorney-at-law, 12020 Shamrock Plaza Suite 200, Omaha, NE 68145, (402) 235-1112, does hereby provide the following notice of a proposed stepparent adoption:

John Doe: You are hereby notified that P.I.M. plans to consent to the stepparent adoption of her son born to you and her on October 17, 2017 in Cebu City, Philippines. If you are the biological father, you may (i) deny paternity; (ii) waive any parental rights you may have; (iii) relinquish and consent to the adoption; (iv) file a Notice of Objection to Adoption and Intent to Obtain Custody any time during the pregnancy or as late as ten business days after birth pursuant to section 43-104.02 if you are a putative father, or (v) object to the adoption in court within forty-five days after the later of receipt of notice under this section or the birth of the child if you are an acknowledged or adjudicated father.

If you wish to deny paternity, waive your parental rights, relinquish and consent, or receive additional information to determine whether you may be the father of the child in question, you must contact Lucrece H. Bundy at the above address and/or phone number. If you wish to object to the adoption and seek custody of the child, you must seek legal counsel from your own attorney immediately.
